In probably well over 100,000 Prutot that we have handled, I cannot recall seeing an error like this. The coin has has a brockage (A brockage is when in the impression is in incuse instead of in relief due to a coin not having been removed from the die, and another coin is struck on top of it) AND is double-struck too!
